Frequently Asked Questions

How do I start my enrollment?

From the email or postcard you received, navigate to the program information page using the link provided. From this program information page, you can start your program enrollment online by entering your information through a secure web form. You can also call NourishedRx using the phone number provided to start your enrollment process. NourishedRx will verify your eligibility with Blue Cross NC, ask you to answer a few questions, and may request that you upload a copy of your most recent hemoglobin A1C (HbA1c) lab result (taken in the last six months). Your provider can also fax your most recent A1C lab result directly to us at 917-540-2450. Once this information has been provided, NourishedRx will review and notify you of your program qualification status and next steps to complete enrollment within three business days.

What if I receive an error message when I try to register?

Ensure you are entering your 2025 member or subscriber ID number including the prefix and last two digits on the card, as applicable. NourishedRx will verify the ID for members who received invitations to participate. If you did not receive an invitation, we will not be able to recognize the ID number provided.

How do I know if I am eligible to participate in the program?

To be eligible to participate in the program, members must be
1) currently enrolled in an eligible Blue Cross NC health plan. Some plan exclusions apply
2) age 18 and older
3) be diagnosed with type 2 diabetes
4) have an HbA1c value of 8.0% or greater in the last six months

and do not meet any of the following exclusion criteria: liquid diet, stage 5 chronic kidney disease, end-stage renal disease, renal replacement or dialysis, enrolled in hospice, or living in a nursing or long term care facility.

What information am I required to provide to participate in the program?

As part of the enrollment process, you will be required to provide your name, date of birth, health plan Member ID available from your Blue Cross NC Member ID card, contact information, and you may be required to provide a copy of your most recent HbA1c lab result (taken in the last 6 months).

Why might I need to provide an HbA1c lab result?

The program is intended to support members with uncontrolled diabetes. The HbA1c indicates the average of blood sugar values over a three month period. HbA1c values of 8 and greater are considered uncontrolled. To participate, you may be required to upload an HbA1c lab result taken in the last six months, and have another value taken approximately four months after program enrollment to help assess the impact of the program

How do I obtain my HbA1c lab result?

If you are having trouble accessing your HbA1c lab result through your online health records, please contact your primary care provider. Your provider can fax or securely email your most recent A1C lab result directly to us at 917-540-2450 or feedyourhealth@nourishedrx.com. If you do not have a primary care provider, please call Blue Cross NC customer service using the number listed on the back of your Member ID card and they will assist you, or log into Blue Connect to find a primary care provider in your network (https://member.bcbsnc.com/blueconnect/web/login)

How much does an HbA1c lab test cost?

Labs will be subject to deductible/co-insurance based on your laboratory benefit within the benefit booklet. HbA1c tests should be taken every three months if your blood sugar is not controlled.

What information is required to be on the HbA1c lab result I provide?

Please ensure the copy of your lab result includes your first and last name, date of birth, HbA1c value, and the date of the lab test.

How does the program work?

Once you are enrolled in the program, you will complete an intake assessment and schedule your first nutrition coaching session with a NourishedRx registered dietitian. You will receive your first healthy home-delivered food after completing your intake assessment, and will continue to receive food for 4 months. You will engage in nutrition coaching sessions with a NourishedRx registered dietitian and you will have additional support from a NourishedRx wellness associate for 6 months.

How will I work with the registered dietitian?

You will meet with the registered dietitian telephonically on an ongoing basis over a 6 month period. The registered dietitian will work with you to understand your health, motivations, and challenges, and take you through an education curriculum tailored to your needs and goals. The registered dietitian will facilitate balanced and nutrient-rich home-delivered food on your behalf, accounting for your dietary restrictions, food preferences, and feedback on previous deliveries.

How will I work with the wellness associate?

Your wellness associate will help you with enrollment and continued engagement in the program, answer any questions you might have about the program, encourage and motivate you throughout the program, and help connect you to Blue Cross NC or community-based resources that might be helpful to you.

What types of food will I receive?

You will receive a combination of healthy prepared meals, meal kits, and groceries throughout the first 4 months of the program. The food will be tailored to your dietary needs and preferences. Your registered dietitian will facilitate food delivery on your behalf.

How long does the program last?

After you enroll in the program, you will receive food for 4 months, receive support from a dietitian and wellness associate for 6 months, and have access to online resources for up to 12 months. Please note that this program can change or be eliminated at any time.

Can I re-enroll after I’ve completed the program?

No - you are eligible to participate one time.

Is the food actually free?

Yes, the combination of healthy food, nutrition and resources are offered at no additional cost to you.

When will I begin receiving the food?

You will receive your first healthy home-delivered food after completing your intake assessment, and will continue to receive food for 4 months. You will receive order and delivery notifications for each food order.

Can I order my own food?

No - you can provide input and feedback on the food you receive, but your registered dietitian will carefully select balanced and nutrient-rich food on your behalf.

Where does the food come from?

NourishedRx partners with a variety of food suppliers, including national and local suppliers, to deliver healthy meals and groceries.


What if I don’t like the food?

You will have an opportunity to rate the food you receive throughout the program through surveys, and you can also share feedback with your registered dietitian and wellness associate. The registered dietitian you work with will incorporate your feedback into future food deliveries to best meet your needs and preferences.

How will I know when the food will be delivered?

You will receive food order and delivery notifications through email, text, phone, and/or the NourishedRx app - whichever you prefer.

Why was I selected for this program?

Maintaining a healthy diet is a key to living well with diabetes. We know it can sometimes be challenging to consistently access and eat healthy foods, while maintaining other aspects of your life and health. This program is designed to support healthy eating and healthy living to improve confidence and quality of life with diabetes.

What if my neighbor or spouse is interested?

Only eligible members who have been invited to participate are eligible for the program.
